Please address the following items/questions in your submission. It is not mandatory that you provide everything outlined below, but the more information you can provide, the easier it will be for the judges to assess your entry. Submissions can include printed brochures, DVD, CD-ROM, or video presentations. As it is difficult to circulate DVDs or videotapes to judges, it is most helpful if you can provide links to a Web site for video presentations.

  • Overview/Background
Provide a brief overview of the nomination. When was the facility/device/product created? When was it (or when will it be) commercially available or otherwise made available to the public?

  • Innovation/Technical Creativity
How does the nominee demonstrate innovation/technical creativity? Is this a completely new facility/device/product or a new application of existing technology? How does it differ from currently available facilities/devices/products?

  • Business Impact
If the nominee is publicly available, what is the price? How is it available to users?

  • Social Impact
How does the nominee improve the lives of those with disabilities? Who is the target user? How does the nominee go beyond the regulatory requirements of the Americans with Disabilities Act (or similar foreign legislation)?

  • Universal Design
Show how the nominee demonstrates the principles of universal design, which can be found on the website of the Center for Universal Design at North Carolina State University (http://design.ncsu.edu/cud/about_ud/about_ud.htm)
